Nursing Homes and Assisted Living Facilities

Thank you for your interest in volunteering at a nursing home or assisted living facility. Please follow these steps to get started.

Step 1: Choose a specific nursing home or assisted living facility

Please visit the DADS Quality Reporting System (QRS) website to learn more about nursing homes and assisted living facilities in your area:

You may enter a city, ZIP code or other search criteria on the QRS site. The rating scale provides additional information that may help you narrow your selection to ensure a good match for you or your volunteer group.

If you have additional questions about how to choose a facility, contact the DADS Volunteer and Community Engagement Unit at 1-800-889-8595.

Step 2: Contact the activities director

Once you have selected a nursing home or assisted living facility, contact the activities director. This person will be the best resource for new volunteers.

The activities director will be able to give you information on:

  • Volunteer requirements. Each facility has specific policies and procedures.
  • Scheduling your volunteer visit
  • Information about your activity ideas
  • A “wish list” for additional activities at that center

Step 3: Register as a volunteer

Click here to complete the DADS volunteer application

After speaking with the activities director about your volunteer goals, complete the Volunteer Application, Form 8653, to officially register as a volunteer. You can get to the application by clicking on the button on this page. Please fax your completed volunteer application to 512-438-4829 or e-mail your completed application to Volunteerdads@dads.state.tx.us.

Please note:

  • You will need to have the activity director’s name and e-mail address or phone number to complete the registration form.

  • Please be sure to check the Silver Lining option in the "Volunteer Interest/Background" section of the form.

As a registered Silver Lining volunteer/volunteer group, you will receive updates on Silver Lining events, volunteer resources, and other volunteer incentives for participating in the program.
